Massage Intake Forms: Share What Is Relevant

An intake process should gather information needed for care while offering a usable way to provide it.

Ask why information is requested

An RMT needs relevant health and access information to assess and plan care. CMTO’s current information-collection standard emphasizes reasonable need and different collection methods. If a question is unclear, ask how it relates to treatment. Do not guess at an answer you do not understand simply to make an online form submit.

Request the format you can use

You may need a verbal conversation, a structured digital form, larger text or help from an agreed support person. Ask how that can be arranged without discussing private details in reception. Complete what you can accurately and tell the practitioner what remains uncertain. A form is a starting point for conversation, not a substitute for it.

Update meaningful changes

Tell the practitioner about relevant changes since the previous visit, including access needs or a new concern. A prefilled old form may no longer describe the current situation. Ask who can see the record and how to correct an error. Keep administrative contact preferences distinct from permission to disclose your health information to relatives, a hotel or a booking platform.

A practical comparison

Hypothetical planning example: A client cannot use a long online form but can answer relevant questions verbally in a private room. That is a format issue, not evidence that the client is unwilling to provide information. Arrange sufficient time and explain what is needed. A relative should not fill in guesses simply to complete required fields. If a detail is unknown, identify it as unknown and discuss whether care can proceed or whether another source of information is needed.
